Overview

Ibrutinib + pembrolizumab is an investigational combination therapy consisting of two distinct agents with complementary mechanisms of action. Ibrutinib is a first-in-class, orally administered small molecule inhibitor of Bruton's Tyrosine Kinase (BTK) and also inhibits Interleukin-2-inducible T-cell Kinase (ITK), modulating B-cell receptor signaling and skewing immune responses toward a Th1 phenotype. Pembrolizumab is a humanized monoclonal antibody that targets the programmed cell death protein 1 (PD-1) receptor, functioning as an immune checkpoint inhibitor to enhance anti-tumor T-cell activity. The rationale for combining these drugs lies in the potential for synergistic anti-cancer effects by both directly inhibiting tumor cell survival pathways and enhancing anti-tumor immunity. This combination has been evaluated in phase 1/2 clinical trials for metastatic colorectal cancer, chronic lymphocytic leukemia, mantle cell lymphoma, advanced melanoma, and other hematologic malignancies[1][2][3][6].
