Drug intelligence / Profile preview
Ibudilast is an orally active, brain-penetrant small molecule that acts as a multi-target phosphodiesterase (PDE) inhibitor and neuroimmune modulator. It selectively inhibits PDE3, PDE4, PDE10, and PDE11, while also acting as an allosteric inhibitor of macrophage migration inhibitory factor (MIF) and an antagonist of Toll-like receptor 4 (TLR4). Originally developed and approved in Japan and South Korea for the treatment of asthma, allergic conjunctivitis, and post-stroke dizziness, ibudilast is being repurposed for neurodegenerative and neuroinflammatory conditions. It crosses the blood-brain barrier to suppress glial cell activation, reduce pro-inflammatory cytokine production, and promote neurotrophic factors. Clinical development is ongoing for amyotrophic lateral sclerosis (ALS), progressive multiple sclerosis (MS), degenerative cervical myelopathy (DCM), glioblastoma, Long COVID, and substance use disorders.
