Ibuprofen + gabapentin is a combination therapy consisting of the nonsteroidal anti-inflammatory drug (NSAID) ibuprofen and the gabapentinoid anticonvulsant gabapentin. Ibuprofen acts as a non-selective inhibitor of cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2) enzymes, which reduces the synthesis of prostaglandins involved in pain and inflammation. Gabapentin binds with high affinity to the alpha-2-delta-1 subunit of voltage-gated calcium channels in the central nervous system, modulating the release of excitatory neurotransmitters such as glutamate and substance P. This combination is primarily investigated for the management of postoperative pain, neuroinflammation, and anxiety, particularly following surgical procedures like the removal of impacted mandibular third molars. By targeting both peripheral inflammatory pathways and central pain processing mechanisms, the combination aims to provide superior analgesia compared to monotherapy.
