Overview

This is a combination of four analgesic drugs—ibuprofen, oxycodone, acetaminophen, and hydromorphone—used for pain management. Ibuprofen is a nonsteroidal anti-inflammatory drug (NSAID) that reduces inflammation and pain by inhibiting cyclooxygenase (COX) enzymes. Oxycodone and hydromorphone are opioid agonists that act primarily on mu-opioid receptors in the central nervous system to provide potent analgesia; both are used for moderate to severe pain but differ in potency and pharmacokinetics. Acetaminophen is an analgesic and antipyretic agent with a mechanism not fully understood but believed to involve inhibition of prostaglandin synthesis in the CNS. While combinations of two or three of these agents (such as oxycodone with acetaminophen or ibuprofen) are approved for acute pain management, there is no known marketed product or clinical precedent for combining all four agents together into a single formulation due to increased risk of adverse effects such as respiratory depression, hepatotoxicity, nephrotoxicity, gastrointestinal bleeding, sedation, and potential drug interactions[1][4][5][6][7].
