This combination therapy consists of icaritin, a small molecule phytochemical with immunomodulatory properties, administered alongside standard-of-care AG chemotherapy, which comprises gemcitabine and nab-paclitaxel. Icaritin, derived from the *Epimedium* plant, is an oral agent that has been shown to downregulate pro-inflammatory cytokines such as TNF-α and IL-6 and reduce PD-L1 expression. These actions help modulate the tumor immune microenvironment by inhibiting myeloid-derived suppressor cells (MDSCs) and enhancing anti-tumor immunity. Gemcitabine is a nucleoside analog that inhibits DNA synthesis, while nab-paclitaxel is an albumin-bound nanoparticle formulation of paclitaxel that stabilizes microtubules to prevent cell division. This specific regimen is being investigated by Sir Run Run Shaw Hospital for the treatment of previously untreated advanced pancreatic ductal adenocarcinoma (PDAC), aiming to improve survival outcomes and increase the success rate of conversion therapy.
