Icodextrin + xylitol + l-carnitine is an experimental combination used as an osmotic agent in peritoneal dialysis (PD) solutions designed to replace or substantially reduce glucose exposure in dialysis fluids. Icodextrin, a glucose polymer, supports colloid osmosis for sustained ultrafiltration. Xylitol (a five-carbon sugar alcohol) and l-carnitine (a quaternary ammonium compound involved in fatty acid metabolism) are added as alternative, metabolically active osmolytes with a favorable safety profile. This approach aims to minimize glucose-associated toxicity, maintain or improve fluid and solute removal efficiency, spare metabolic burden, and potentially offer systemic metabolic benefits for dialysis patients. The combination seeks to provide effective ultrafiltration and dialysis performance comparable to standard glucose-based solutions, with improved local biocompatibility and systemic safety.
