ICP-248 + orelabrutinib is a combination therapy under clinical investigation for the treatment of hematologic malignancies, primarily chronic lymphocytic leukemia (CLL) and small lymphocytic lymphoma (SLL). ICP-248 (also known as Mesutoclax) is a novel, orally bioavailable, highly selective B-cell lymphoma 2 (BCL2) inhibitor that promotes apoptosis in cancer cells by blocking BCL2 activity. Orelabrutinib is an oral Bruton’s tyrosine kinase (BTK) inhibitor that disrupts B-cell receptor signaling. The combination aims to achieve deeper remissions in treatment-naïve CLL/SLL patients without drug-resistant mutations and has shown promising efficacy and safety in early-phase trials. Both drugs are developed by InnoCare Pharma, with ongoing Phase III registrational trials for first-line CLL/SLL therapy. Additional studies are evaluating the combination or monotherapy use of ICP-248 in other blood cancers such as acute myeloid leukemia (AML), non-Hodgkin’s lymphoma (NHL), mantle cell lymphoma (MCL), and marginal zone lymphoma (MZL)[1][2][3][5][7].
