Overview

idelalisib + fludarabine is a combination therapy regimen studied for the treatment of relapsed or refractory chronic lymphocytic leukemia (CLL). It consists of idelalisib (Zydelig), a small molecule inhibitor of the delta isoform of phosphatidylinositol 3-kinase (PI3Kδ), and fludarabine (Fludara), a purine nucleoside analog chemotherapy. Idelalisib blocks the PI3Kδ signaling pathway, which is essential for the proliferation and survival of malignant B-cells. Fludarabine acts as a cytotoxic agent by inhibiting DNA synthesis and repair. This combination was evaluated in Phase I clinical trials to assess safety and efficacy in patients who had failed prior therapies, showing high overall response rates, although idelalisib's clinical development in combination with other agents has been complicated by safety signals in later-stage trials.
