Overview

The combination of ifosfamide, carboplatin, and etoposide—commonly referred to as the ICE regimen—is a multi-agent chemotherapy protocol primarily used for cytoreduction and stem cell mobilization in patients with relapsed or refractory lymphomas (notably non-Hodgkin lymphoma) and is also used in small-cell lung cancer and other malignancies. Ifosfamide is an alkylating agent that crosslinks DNA, leading to apoptosis; carboplatin is a platinum-based compound that forms DNA adducts causing cell death; etoposide inhibits topoisomerase II, resulting in DNA strand breaks. The regimen is highly myelosuppressive but effective for tumor reduction prior to autologous stem cell transplantation. It may be administered with supportive agents such as mesna (to prevent hemorrhagic cystitis from ifosfamide) and granulocyte colony-stimulating factor (G-CSF) for hematopoietic support[1][2][3][7][8].
