Overview

This is a high-dose chemotherapy combination regimen consisting of five cytotoxic agents: ifosfamide, carboplatin, etoposide, mitoxantrone, and thiotepa. Each drug has a distinct mechanism of action targeting cancer cells at different stages or via different pathways. Ifosfamide is an alkylating agent that crosslinks DNA; carboplatin is a platinum-based compound causing DNA damage; etoposide inhibits topoisomerase II leading to DNA strand breaks; mitoxantrone intercalates into DNA and inhibits topoisomerase II; and thiotepa is another alkylating agent that crosslinks DNA. This combination has been studied in the context of high-dose therapy for metastatic breast cancer with autologous stem cell rescue to allow recovery from myelosuppression. The regimen aims to maximize antitumor activity by combining drugs with non-overlapping toxicities and mechanisms[1][5].
