Overview

Ifosfamide, etoposide, methotrexate, vindesine, and prednisone is a multi-agent cytotoxic chemotherapy regimen used as a backbone treatment arm (specifically the 'BB course') in clinical protocols developed by the **Chinese Children's Cancer Group (CCCG)**. This combination is primarily indicated for children and adolescents with aggressive **mature B-cell non-Hodgkin lymphoma (B-NHL)**, including advanced-stage disease (Risk Groups 3 and 4). The regimen functions through multiple synergistic mechanisms: **ifosfamide** acts as a DNA alkylating agent; **etoposide** inhibits topoisomerase II; **methotrexate** is a folate antimetabolite that inhibits dihydrofolate reductase; **vindesine** is a vinca alkaloid that disrupts microtubule polymerization; and **prednisone** is a corticosteroid that induces apoptosis in lymphoid cells. In modern clinical trials, such as NCT07168980 (CCCG-BNHL-2025), this intensive chemotherapy backbone is combined with the anti-CD20 monoclonal antibody rituximab to improve event-free and overall survival.
