XNW5004 + docetaxel + prednisone is an investigational combination regimen being evaluated for the treatment of advanced prostate cancer, specifically metastatic castration-resistant prostate cancer (mCRPC). The backbone of the regimen is igermetostat (XNW5004), a novel, selective, substrate-competitive small molecule inhibitor of Enhancer of Zeste Homolog 2 (EZH2). EZH2 is the catalytic subunit of the Polycomb Repressive Complex 2 (PRC2), which represses gene transcription through the trimethylation of histone H3 at lysine 27 (H3K27me3). By inhibiting EZH2, igermetostat aims to reverse the epigenetic silencing of tumor suppressor genes. This novel agent is combined with docetaxel, a standard-of-care taxane chemotherapy that stabilizes microtubules to inhibit mitosis, and prednisone, a corticosteroid typically administered alongside docetaxel to manage side effects and provide additional anti-tumor support. The triplet combination is currently being studied in Phase Ib/II clinical trials by Evopoint Biosciences to assess its safety, tolerability, and clinical efficacy.
