IL-2 + nivolumab is a combination immunotherapy regimen consisting of interleukin-2 (IL-2), a cytokine that stimulates the proliferation and activation of T cells and natural killer (NK) cells, and nivolumab, a monoclonal antibody that inhibits programmed cell death protein 1 (PD-1), thereby enhancing antitumor immune responses. This combination has been explored in clinical settings for patients with advanced or metastatic melanoma, particularly those refractory to prior anti-PD-1 therapies. The rationale is that high-dose IL-2 can further activate immune effector cells even in the context of PD-1 resistance, while nivolumab blocks inhibitory signaling on T cells to sustain their activity against tumor cells. Early case reports suggest tolerability similar to each agent alone, but large-scale efficacy data are limited[2]. Modified forms of IL-2 such as bempegaldesleukin have also been combined with nivolumab in clinical trials for melanoma and renal cell carcinoma; however, these combinations did not show significant added benefit over monotherapy with increased toxicity[1][3][7].
