Iloprost + eptifibatide is an experimental combination therapy consisting of two agents with complementary mechanisms. Iloprost is a synthetic prostacyclin analog that acts as a potent vasodilator and inhibitor of platelet aggregation, primarily used in pulmonary arterial hypertension and frostbite. Eptifibatide is a cyclic heptapeptide derived from snake venom that functions as a selective, reversible antagonist of the platelet glycoprotein IIb/IIIa receptor, inhibiting platelet aggregation and used mainly in acute coronary syndromes and percutaneous coronary intervention. The combination has been studied experimentally for its potential to reduce endothelial injury, modulate inflammation, decrease platelet consumption, and improve outcomes in septic shock by targeting both endothelial activation (via iloprost) and platelet function (via eptifibatide)[1][4]. This dual approach aims to interrupt the cycle of microvascular injury characteristic of severe sepsis.
