This combination therapy consists of oral imatinib mesylate (Gleevec) and intravitreal ranibizumab (Lucentis). It was investigated in a Phase 1 clinical trial sponsored by Vitreous-Retina-Macula Consultants of New York for the treatment of newly diagnosed choroidal neovascularization (CNV) secondary to age-related macular degeneration (AMD). Imatinib is a small molecule tyrosine kinase inhibitor that targets platelet-derived growth factor receptors (PDGFR), which are involved in pericyte recruitment and stabilization of blood vessels. Ranibizumab is a monoclonal antibody fragment that inhibits vascular endothelial growth factor A (VEGF-A), a key driver of angiogenesis and vascular permeability. The rationale for the combination is to simultaneously inhibit VEGF-mediated vessel growth and PDGF-mediated pericyte protection of those vessels, potentially enhancing the regression of neovascular membranes compared to anti-VEGF monotherapy.
