Imetelstat + paclitaxel is an investigational combination therapy studied primarily for the treatment of locally recurrent or metastatic breast cancer. Imetelstat is a first-in-class, 13-mer lipidated oligonucleotide that acts as a telomerase inhibitor by binding to the template RNA component of telomerase, thereby inhibiting its activity and targeting cancer cell self-renewal and proliferation. Paclitaxel is a well-established chemotherapeutic agent that stabilizes microtubules by binding to β-tubulin, preventing their depolymerization and thus inhibiting mitosis in rapidly dividing cells. The combination has been evaluated in phase 1 and phase 2 clinical trials for efficacy and safety, with preliminary results showing objective responses but also increased hematologic toxicities such as neutropenia[1][2][5].
