Imipenem + cilastatin + fosbactam is a novel triple-combination antibiotic therapy developed by Suzhou Sinovent Pharmaceutical. It is designed to combat multi-drug resistant Gram-negative bacterial infections, particularly those caused by carbapenem-resistant Enterobacteriaceae (CRE) and *Pseudomonas aeruginosa*. The combination consists of **imipenem**, a broad-spectrum carbapenem antibiotic that inhibits bacterial cell wall synthesis; **cilastatin**, a renal dehydropeptidase-1 inhibitor that prevents the metabolic degradation of imipenem in the kidneys; and **fosbactam** (XNW4107), a novel diazabicyclooctane (DBO) beta-lactamase inhibitor. Fosbactam effectively inhibits a wide range of beta-lactamases, including Class A, Class C, and certain Class D enzymes (such as OXA-48), thereby restoring the efficacy of imipenem against resistant strains. The therapy is currently in Phase 3 clinical development for the treatment of complicated urinary tract infections (cUTI), hospital-acquired pneumonia (HAP), and ventilator-associated pneumonia (VAP).
