Overview

**Imipenem + cilastatin + relebactam** is an intravenous fixed-dose antibacterial combination marketed as **Recarbrio**. Imipenem is a carbapenem that inhibits bacterial cell-wall synthesis through penicillin-binding-protein inhibition; cilastatin inhibits renal dehydropeptidase 1, preventing renal metabolism of imipenem; and relebactam is a diazabicyclooctane beta-lactamase inhibitor that protects imipenem from susceptible class A and class C serine beta-lactamases, including Klebsiella pneumoniae carbapenemase. Developed and marketed by Merck, it is approved for susceptible Gram-negative infections including hospital-acquired bacterial pneumonia, ventilator-associated bacterial pneumonia, complicated urinary tract infection including pyelonephritis, and complicated intra-abdominal infection in defined patient populations.
