Overview

This is a combination of two pharmaceutical drugs, imipramine hydrochloride and lithium carbonate, used primarily in the management of mood disorders such as bipolar disorder and recurrent unipolar depression. Imipramine is a tricyclic antidepressant that increases synaptic concentrations of norepinephrine and serotonin by inhibiting their reuptake in the central nervous system. Lithium carbonate is a mood stabilizer whose precise mechanism remains unclear but is thought to involve inhibition of enzymes such as glycogen synthase kinase 3 (GSK-3) and inositol monophosphatases, affecting second messenger systems within neurons. The combination has been studied for its efficacy in preventing depressive relapses or affective episodes; however, evidence suggests that while both agents are effective individually for certain populations (e.g., unipolar depression for both; bipolar disorder mainly for lithium), the combination does not provide significant additional benefit over monotherapy with lithium. The risk profile may include an increased risk of manic episodes when combined, particularly in mania-prone patients[1][3][6]. This regimen should be used under close medical supervision due to potential drug interactions and side effects[5].
