Overview

Imiquimod + sargramostim is a combination of two immunomodulatory agents. Imiquimod is an immune response modifier that acts as a toll-like receptor 7 agonist, stimulating both innate and adaptive immune responses. It induces cytokine production (including interferon-alpha, interleukins, and tumor necrosis factor alpha), leading to inflammatory cell infiltration and apoptosis of diseased tissue. Imiquimod is primarily used topically for actinic keratosis, superficial basal cell carcinoma, and external genital/perianal warts[2][3][4]. Sargramostim is a recombinant human granulocyte-macrophage colony-stimulating factor (GM-CSF) that stimulates the proliferation and differentiation of hematopoietic progenitor cells into granulocytes and macrophages; it also enhances the function of mature neutrophils, monocytes/macrophages, and dendritic cells[5]. Sargramostim (marketed as Leukine) is used to accelerate myeloid recovery after bone marrow transplantation or chemotherapy-induced neutropenia, in high-risk neuroblastoma regimens with anti-GD2 antibodies[5], and for acute radiation syndrome. The combination may be explored in investigational immunotherapy regimens where enhanced local immune activation by imiquimod could be synergistically supported by systemic or local GM-CSF from sargramostim to further recruit/activate antigen-presenting cells.
