INCB053914 + cytarabine is a combination investigational therapy studied primarily for the treatment of acute myeloid leukemia (AML), particularly in patients who have relapsed, are refractory, or are not candidates for intensive chemotherapy. INCB053914 is a selective, oral pan-PIM kinase inhibitor designed to block PIM kinases (PIM1, PIM2, PIM3), which are serine/threonine kinases implicated in cancer cell proliferation, survival, and resistance to apoptosis. Cytarabine is a well-established cytotoxic nucleoside analog used as standard therapy in AML and other hematologic malignancies; it interferes with DNA synthesis and induces cell death in rapidly dividing cells. In clinical studies, the combination of INCB053914 and cytarabine resulted in limited clinical responses, with observed dose-limiting toxicities including rash, elevated liver enzymes, and hypophosphatemia. The combination targets overlapping and complementary pathways involved in leukemic cell survival; INCB053914 inhibits PIM-driven oncogenic signaling, while cytarabine induces DNA damage-mediated cell death[1][7].
