111In-pertuzumab is a radiopharmaceutical diagnostic agent consisting of the recombinant humanized monoclonal antibody pertuzumab labeled with the gamma-emitting radioisotope indium-111 ($^{111}$In). Pertuzumab specifically targets the extracellular dimerization domain (subdomain II) of the human epidermal growth factor receptor 2 (HER2), a mechanism that prevents HER2 from forming heterodimers with other members of the HER family, such as HER3. By conjugating pertuzumab with a chelating agent (typically DOTA or DTPA) and labeling it with indium-111, the agent enables non-invasive visualization and quantification of HER2 expression in vivo using single-photon emission computed tomography (SPECT) imaging. This diagnostic tool is primarily utilized in clinical research to identify HER2-positive lesions in patients with breast or gastric cancer, helping to predict response to HER2-targeted therapies and monitor molecular changes in tumor expression over time.
