Indium-111 MX-DTPA BrE-3 + Yttrium-90 MX-DTPA BrE-3 is an investigational combination therapy for radioimmunotherapy, primarily targeting metastatic and advanced breast cancer. The treatment uses the murine IgG1 monoclonal antibody BrE-3, which binds to 97% of human ductal breast cancer specimens. The antibody is conjugated via MX-DTPA (a chelator) to either indium-111 (a diagnostic gamma-emitting radionuclide for imaging and dosimetry) or yttrium-90 (a therapeutic beta-emitting radionuclide for radioimmunotherapy). Indium-111 is used initially to assess biodistribution and tumor targeting, while yttrium-90 provides a therapeutic dose of targeted radiation. The combination delivers localized cytotoxic radiation to tumors expressing the BrE-3 antigen, aiming to spare healthy tissue. Early-phase clinical trials have shown evidence of tumor response with manageable toxicity, primarily myelosuppression. However, immunogenicity (human anti-mouse antibody response) limits repeated dosing unless mitigated by immunosuppression or antibody humanization[1][3][4][5].
