Overview

This drug is a **combination radiopharmaceutical diagnostic agent** composed of indium in-111 pentetreotide, a somatostatin analog chelated with the radioisotope indium-111, and indium chloride in-111 as the radioisotope source. Indium in-111 pentetreotide is designed to bind to somatostatin receptors, particularly subtype 2, which are overexpressed on certain neuroendocrine tumors and other malignancies. After intravenous administration, the complex is selectively taken up by tissues expressing these receptors, allowing gamma camera imaging to detect and localize tumors. The product is primarily used in nuclear medicine for **tumor localization, staging, and diagnostic imaging** of somatostatin receptor–positive tumors such as neuroendocrine tumors (e.g., carcinoid tumors, insulinomas, gastrinomas, pheochromocytomas, paragangliomas, and others). Indium chloride in-111 is included as a precursor to provide the radioactive isotope required for labeling the pentetreotide ligand at the time of preparation. Octreoscan was developed originally by Mallinckrodt and has been subsequently manufactured by companies specializing in radiopharmaceuticals. This test is administered intravenously and images are acquired using specialized nuclear medicine scans. Mechanism of action: targeting and binding to somatostatin receptor–positive cells, allowing radiolocalization via emission of gamma photons from indium-111 decay[1][3][5].
