This combination therapy regimen consists of inetetamab, rapamycin (sirolimus), and chemotherapy. Inetetamab is an innovative, Fc-engineered recombinant anti-HER2 humanized monoclonal antibody designed to enhance antibody-dependent cellular cytotoxicity (ADCC) compared to standard trastuzumab. Rapamycin is a small molecule inhibitor of the mammalian target of rapamycin (mTOR), which targets the PI3K/Akt/mTOR signaling pathway often implicated in resistance to HER2-targeted therapies. The regimen is currently being evaluated in a Phase 3 clinical trial (NCT04736589) sponsored by Peking Union Medical College for patients with HER2-positive metastatic breast cancer who have progressed on trastuzumab and possess mutations or abnormal activation in the PI3K/Akt/mTOR pathway.
