Overview

Infliximab + tacrolimus is a combination therapy consisting of two immunosuppressive agents used primarily in the management of severe or refractory ulcerative colitis (UC). Infliximab is a chimeric monoclonal antibody that targets and neutralizes tumor necrosis factor alpha (TNF-α), thereby disrupting pro-inflammatory signaling pathways. Tacrolimus is a calcineurin inhibitor that suppresses T-cell activation by inhibiting interleukin-2 transcription through binding to FKBP-12, ultimately blocking the phosphatase activity of calcineurin. This combination has been explored as an option for patients with UC who are refractory to monotherapy or standard escalation regimens, particularly when other treatments such as azathioprine have failed. The combined use aims to induce remission in difficult-to-treat cases, but further studies are needed to fully establish its efficacy and safety profile[1][2].
