Overview

This combination therapy is an experimental clinical intervention being studied by Fayoum University for the treatment of temporomandibular joint (TMJ) anterior disc displacement with reduction (ADDwR). The protocol involves TMJ arthrocentesis (joint lavage) followed by the intra-articular injection of injectable platelet-rich fibrin (I-PRF) and an intramuscular injection of botulinum toxin type A (Botox) into the lateral pterygoid muscle. I-PRF is an autologous biologic derived from the patient's own blood that contains a high concentration of platelets and leucocytes within a fibrin matrix. It acts as a reservoir for growth factors, including platelet-derived growth factor (PDGF), transforming growth factor beta (TGF-β), and vascular endothelial growth factor (VEGF), which promote tissue regeneration and reduce inflammation within the joint space. Botulinum toxin type A is a neurotoxin that inhibits the release of acetylcholine at the neuromuscular junction, leading to temporary relaxation of the lateral pterygoid muscle. This combined approach aims to reduce mechanical strain on the joint while simultaneously enhancing the healing of internal joint structures to improve clinical outcomes such as pain reduction and increased mouth opening.
