This is a combination of two drugs, insulin and octreotide, used together in specific clinical scenarios. Insulin is a peptide hormone that regulates blood glucose by promoting cellular uptake of glucose and inhibiting hepatic glucose production. Octreotide is a synthetic somatostatin analog that inhibits the secretion of several hormones including growth hormone, glucagon, and insulin itself. The combination may be used in rare cases such as patients with acromegaly who also have insulin-requiring diabetes mellitus or for managing complex endocrine tumors affecting both growth hormone and glucose metabolism. Octreotide acts primarily by binding to somatostatin receptors (mainly SSTR2 and SSTR5), leading to inhibition of growth hormone release as well as modulation of gastrointestinal hormones[1][2][3]. When combined with insulin therapy, careful monitoring is required due to potential alterations in glycemic control; octreotide can reduce endogenous insulin secretion or alter counter-regulatory hormonal balance[5].
