insulin + semaglutide is a combination therapy being investigated for the treatment of double diabetes, a clinical phenotype where patients with type 1 diabetes also exhibit insulin resistance typically associated with type 2 diabetes. This regimen involves the addition of weekly semaglutide, a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist, to a patient's standard insulin therapy. Semaglutide works by activating the GLP-1 receptor, which enhances glucose-dependent insulin secretion (in remaining beta cells), suppresses inappropriate glucagon secretion, slows gastric emptying, and promotes weight loss and improved insulin sensitivity. The exogenous insulin component provides the necessary basal and bolus hormone replacement for glucose uptake. The combination is intended to improve overall glycemic control and reduce the total daily insulin requirement in patients with this dual pathology. This specific therapeutic strategy is currently being evaluated in a Phase 3 clinical trial sponsored by the Centre Hospitalier Universitaire Dijon.
