Overview

This is a combination therapy consisting of three classes of antidiabetic agents used in the management of diabetes mellitus, particularly type 2 diabetes. The components are: - **Insulin**: A peptide hormone that lowers blood glucose by facilitating cellular uptake and utilization of glucose, and inhibiting hepatic glucose production. - **Sulfonylurea**: A class of oral hypoglycemic agents that stimulate pancreatic beta cells to secrete insulin by closing ATP-sensitive potassium channels in the cell membrane. - **Meglitinide**: Another class of oral hypoglycemic agents that also stimulate rapid, short-term insulin secretion from pancreatic beta cells via a similar mechanism as sulfonylureas but with different binding sites and pharmacokinetics. The combination aims to maximize glycemic control through complementary mechanisms—directly supplying exogenous insulin while also stimulating endogenous insulin release both over several hours (sulfonylureas) and acutely around meals (meglitinides). This approach may be considered for patients with type 2 diabetes who have not achieved adequate glycemic control on monotherapy or dual therapy[2][3][5][8].
