Overview

Insulin glargine + gliclazide is a combination therapy used in the management of type 2 diabetes mellitus, particularly in patients who are inadequately controlled with oral antidiabetic agents alone. Insulin glargine is a long-acting human insulin analog that provides basal glycemic control by stimulating peripheral glucose uptake and inhibiting hepatic glucose production. Gliclazide is a second-generation sulfonylurea that lowers blood glucose primarily by stimulating pancreatic β-cell insulin secretion through closure of ATP-sensitive potassium channels on the β-cell membrane. The combination aims to improve glycemic control more effectively than either agent alone or premixed insulins, as demonstrated in clinical studies showing greater reductions in HbA1c and fasting plasma glucose without increased risk of hypoglycemia compared to monotherapy[1][2][5]. This regimen leverages both exogenous (insulin) and endogenous (sulfonylurea-stimulated) mechanisms for optimal blood sugar regulation.
