Overview

This is a combination of two distinct insulins used in the management of diabetes mellitus. Insulin lispro is a rapid-acting human insulin analog designed for quick absorption and short duration of action, typically administered before meals to control postprandial blood glucose spikes. Insulin ultralente is an extended (long)-acting form of human insulin that provides basal glycemic control throughout the day and night. When used together, this combination aims to mimic physiological insulin secretion by providing both immediate prandial coverage (via lispro) and sustained basal activity (via ultralente). The mechanism of action for both components involves binding to the insulin receptor, stimulating cellular uptake of glucose in muscle and adipose tissue, inhibiting hepatic glucose production, promoting glycogen synthesis, enhancing protein synthesis, and suppressing lipolysis[1][2][3][4][5][7]. This regimen has been studied for its efficacy in stabilizing postprandial glycemia while maintaining overall glycemic control in patients with type 1 or type 2 diabetes[3].
