**insulin lispro low mixture + metformin** is a fixed combination therapy used primarily for treating type 2 diabetes mellitus in patients who require intensification beyond basal insulin and/or oral antihyperglycemic agents. Insulin lispro low mixture (known as LM25 or Mix 75/25) consists of 75% insulin lispro protamine suspension (an intermediate-acting form) and 25% insulin lispro solution (a rapid-acting form), administered subcutaneously twice daily. The combination improves glycemic control (HbA1c and postprandial glucose)[1][2]. Primary indication is **type 2 diabetes mellitus** inadequately controlled with oral therapeutic agents or basal insulin.
