Insulin ultralente + regular insulin is a combination of two types of human insulins used in the management of diabetes mellitus. Insulin ultralente is an ultra long-acting, zinc-based crystalline suspension that provides basal (background) glycemic control, with an onset of 2–4 hours, a broad peak at 8–16 hours, and duration up to 24 hours. Regular insulin is a short-acting form that begins working within 30 minutes to one hour after injection, peaks at about 2–5 hours, and lasts for up to six hours. The combination regimen aims to mimic physiological insulin secretion by providing both basal and prandial (mealtime) coverage. This approach helps improve glycemic control in patients with type 1 or type 2 diabetes who require both background and mealtime glucose regulation[3][5][9]. Both components act as agonists at the insulin receptor, facilitating cellular uptake of glucose from the bloodstream.
