Interferon alfa refers to a family of recombinant or naturally occurring cytokines with antiviral and immunomodulatory properties. Pegylated interferon alfa (peginterferon alfa) is a modified form in which polyethylene glycol (PEG) is covalently attached to the interferon molecule. This modification extends the half-life and allows for less frequent dosing compared to conventional (non-pegylated) interferons. Both forms act by binding to type 1 human interferon receptors on cell surfaces, activating the JAK/STAT signaling pathway and inducing expression of genes involved in antiviral defense and immune regulation. These drugs have been foundational therapies for chronic hepatitis C virus (HCV) infection—often in combination with ribavirin—and are also used for chronic hepatitis B virus infection[1][2][3][8]. The introduction of direct acting antivirals has reduced their use as first-line therapy.
