This investigational immunotherapy combination regimen consists of recombinant interferon gamma-1b (marketed as Actimmune) and donor leukocyte infusion (DLI). Developed by Dr. Sawa Ito at the UPMC Hillman Cancer Center, the treatment is designed for patients with relapsed acute myeloid leukemia (AML) or myelodysplastic syndromes (MDS) following allogeneic hematopoietic stem cell transplantation (alloSCT). The therapeutic mechanism involves the administration of interferon-gamma to upregulate HLA expression and other interferon-inducible molecules on the surface of leukemia blasts. This process sensitizes the malignant cells to alloreactive T-cell-mediated killing, thereby enhancing the graft-versus-leukemia (GVL) effect when combined with the subsequent infusion of donor-derived lymphocytes. A Phase 1 pilot trial demonstrated safety and high remission rates, leading to an ongoing Phase 2 trial to confirm efficacy.
