Interferon regulatory factor 5 (IRF5) messenger RNA nanoparticles + I-kappa-B kinase beta (IKKβ) messenger RNA nanoparticles is an experimental combination immunotherapy designed to reprogram tumor-associated macrophages (TAMs). TAMs typically exhibit an immunosuppressive M2 phenotype that promotes tumor growth. This therapy uses targeted nanocarriers to deliver in vitro-transcribed mRNA encoding the M1-polarizing transcription factor IRF5 and its activating kinase IKKβ directly to TAMs. By expressing these factors, the macrophages are shifted toward a pro-inflammatory M1 phenotype, which triggers anti-tumor immunity and promotes tumor regression. Preclinical studies have demonstrated efficacy and safety in models of ovarian cancer, melanoma, and glioblastoma.
