Intismeran + pembrolizumab is an investigational combination cancer immunotherapy co-developed by Moderna and Merck. Intismeran (also known as intismeran autogene, formerly V940 or mRNA-4157) is an mRNA-based individualized neoantigen therapy (INT) designed to encode up to 34 patient-specific tumor neoantigens, eliciting a targeted T-cell immune response. Pembrolizumab (Keytruda) is a humanized monoclonal antibody that acts as a programmed cell death protein 1 (PD-1) checkpoint inhibitor, preventing PD-1 from interacting with its ligands (PD-L1 and PD-L2) to restore T-cell anti-tumor activity. Together, the combination is designed to synergistically train and activate the patient's immune system to recognize and destroy residual cancer cells. It is primarily being evaluated as an adjuvant therapy for completely resected high-risk melanoma (stages IIB–IV), non-small cell lung cancer (NSCLC), and other solid tumors.
