**Intismeran autogene + BCG** is a combination investigational immunotherapy regimen for high-risk non-muscle invasive bladder cancer (HR NMIBC). Intismeran autogene (V940, also known as mRNA-4157) is a personalized mRNA-based cancer vaccine that encodes patient-specific neoantigens, designed to stimulate the immune system to recognize and attack tumor cells. Bacillus Calmette-Guerin (BCG) is an established live attenuated strain of Mycobacterium bovis used intravesically as immunotherapy in bladder cancer, acting mainly by activating local immune responses in the bladder. In combination, intismeran autogene aims to enhance systemic anti-tumor immunogenicity, while BCG provides potent local immune stimulation, potentially leading to improved outcomes in NMIBC by reducing recurrence and progression rates[2][3][5][6].
