Overview

This combination therapy is an aggressive antiplatelet regimen evaluated in the RESIST trial by the Icahn School of Medicine at Mount Sinai. It consists of an intravenous glycoprotein IIb/IIIa inhibitor (such as abciximab, eptifibatide, or tirofiban) added to standard oral dual antiplatelet therapy (DAPT) comprising acetylsalicylic acid (aspirin) and clopidogrel. The regimen was specifically studied in aspirin-resistant patients with stable angina undergoing percutaneous coronary intervention (PCI) to reduce the risk of periprocedural ischemic events. The GP IIb/IIIa inhibitor blocks the final common pathway of platelet aggregation by targeting the integrin alpha-IIb/beta-3 receptor, while aspirin inhibits cyclooxygenase-1 (COX-1) and clopidogrel antagonizes the P2Y12 receptor.
