Overview

This is a combination therapy consisting of three agents—intravenous immunoglobulin (IVIG), methylprednisolone, and vincristine—used together primarily in the management of certain autoimmune hematologic disorders such as immune thrombocytopenic purpura (ITP) and other severe or refractory immune-mediated cytopenias. - **Intravenous immunoglobulin (IVIG)** is a pooled antibody product derived from human plasma, used to modulate immune responses by providing passive immunity and interfering with autoantibody activity. Its mechanisms include Fc receptor blockade, modulation of complement activation, neutralization of autoantibodies, and regulation of cytokine production[1][2][5]. - **Methylprednisolone** is a synthetic glucocorticoid that suppresses inflammation and modulates the immune response by altering gene expression through glucocorticoid receptor binding. It reduces cytokine production, inhibits leukocyte migration, and decreases capillary permeability[6][8][10]. - **Vincristine** is an antineoplastic agent classified as a vinca alkaloid; it binds to tubulin proteins in microtubules to inhibit mitosis in rapidly dividing cells. In hematologic diseases like ITP, it also impairs macrophage function involved in platelet destruction. This combination leverages complementary mechanisms for rapid control of severe autoimmune cytopenias or when monotherapy has failed.
