Overview

This is a combination therapy consisting of intravenous immunoglobulin (IVIg) and platelet transfusion. IVIg is a blood product containing pooled IgG antibodies from thousands of donors, used to modulate immune responses. Platelet transfusions provide exogenous platelets to rapidly increase circulating platelet counts. This combination is primarily used in the management of severe thrombocytopenia, such as in immune thrombocytopenia (ITP), especially when rapid elevation of platelet count is required to prevent or control bleeding. IVIg works by several mechanisms, including blocking Fc receptors on macrophages to reduce destruction of antibody-coated platelets and possibly stimulating megakaryocytes to increase endogenous platelet production[8]. Platelet transfusions directly supplement the patient’s circulating platelets but are often transiently effective unless the underlying cause of destruction or consumption is addressed. Combination therapy with IVIg and platelet transfusion may be considered in emergency situations (e.g., active bleeding or prior to surgery) where immediate hemostatic correction is needed, although more commonly IVIg is combined with corticosteroids for ITP[1][6][8].
