Overview

Iobenguane (123I), marketed as MIBGen by ANSTO, is a radiopharmaceutical diagnostic agent used for the localization and imaging of neuroendocrine tumors, particularly those of adrenergic origin such as pheochromocytoma, neuroblastoma, and paraganglioma. It is a radioiodinated analog of guanethidine and acts as a structural analog of norepinephrine. The molecule is taken up by the norepinephrine transporter (NET) into presynaptic vesicles of sympathetic nerve endings and chromaffin cells. Once internalized, the Iodine-123 isotope emits gamma radiation, which is detected via Single Photon Emission Computed Tomography (SPECT) to visualize adrenergic tissues and tumors. It is administered intravenously and is primarily used in the assessment of catecholamine-secreting tumors and sympathetic innervation of the heart.
