Iobenguane (131I), also known as 131I-MIBG, is a radiopharmaceutical consisting of the norepinephrine analog iobenguane labeled with the radioisotope iodine-131. Developed by Jubilant DraxImage, it is primarily investigated for the treatment of recurrent or progressive high-risk neuroblastoma. The drug functions by targeting the norepinephrine transporter (NET), which is highly expressed in neuroblastoma and other neuroendocrine tumors. Once internalized into the cells via NET and stored in neurosecretory granules, the iodine-131 isotope emits beta radiation, leading to targeted cytotoxic effects and DNA damage within the tumor cells. It is currently being evaluated in Phase 2 clinical trials, both as a monotherapy and in combination with other agents like the HDAC inhibitor vorinostat.
