Overview

Iodine (124I) evuzamitide (also known as 124I-evuzamitide, AT-01, and 124I-p5+14) is an investigational, first-in-class, radiolabeled synthetic peptide diagnostic imaging agent designed for the detection and quantification of systemic and cardiac amyloidosis. Developed originally by the University of Tennessee and subsequently by Attralus, before being acquired by Bayer in January 2026, the agent consists of a 45-amino-acid synthetic peptide (p5+14) labeled with the positron-emitting isotope iodine-124. As a pan-amyloid binding agent, it targets both amyloid fibrils and associated hypersulfated heparan sulfate proteoglycans (glycosaminoglycans) via multivalent electrostatic interactions. This mechanism allows it to detect multiple types of amyloid deposits, including light-chain (AL) and transthyretin (ATTR) amyloidosis, with high sensitivity and specificity. In clinical trials, including the pivotal Phase III REVEAL study, iodine (124I) evuzamitide has demonstrated strong diagnostic performance for cardiac amyloidosis PET/CT imaging, offering a non-invasive method to diagnose the condition and monitor changes in amyloid burden over time.
