Overview

Iparomlimab + Tuvonralimab + chemotherapy + bevacizumab is a multi-agent neoadjuvant regimen primarily investigated for the treatment of locally advanced rectal cancer (LARC). The regimen's backbone is **QL-1706**, a novel bifunctional 'MabPair' biologic developed by **Qilu Pharmaceutical**, which contains a fixed 2:1 ratio of **iparomlimab** (an anti-PD-1 IgG4 antibody) and **tuvonralimab** (an anti-CTLA-4 IgG1 antibody). This dual checkpoint inhibitor is administered alongside **bevacizumab**, an anti-VEGF monoclonal antibody, and standard **chemotherapy** (typically fluoropyrimidine-based regimens like CAPOX or FOLFOX). By combining dual T-cell activation with anti-angiogenic and cytotoxic effects, the regimen aims to maximize pathological complete response (pCR) rates and potentially facilitate a "watch and wait" approach or the omission of radiotherapy in rectal cancer patients. The regimen is currently being evaluated in the multicenter **IT-CABLE** trial.
