IPI-926 + gemcitabine is a combination therapy under investigation for the treatment of metastatic pancreatic ductal adenocarcinoma and other cancers. IPI-926 (also known as saridegib or patidegib) is an oral small molecule inhibitor that targets the Hedgehog signaling pathway by antagonizing Smoothened (Smo), a key transmembrane protein in this pathway. Inhibition of Smo leads to repression of malignant Hedgehog pathway activation, resulting in decreased tumor desmoplasia and improved delivery of chemotherapeutic agents such as gemcitabine into tumors. Preclinical studies have shown that this combination can break down tumor stroma, increase intratumoral vessel density, enhance chemotherapy delivery, reduce tumor burden, and prolong survival in mouse models. Gemcitabine is a nucleoside analog used as standard chemotherapy for pancreatic cancer. The combination has been evaluated in phase Ib/II clinical trials for safety and efficacy in patients with previously untreated metastatic pancreatic cancer[1][2][4][5][6].
