Overview

This neoadjuvant combination immunotherapy regimen consists of ipilimumab, a monoclonal antibody targeting the CTLA-4 checkpoint, and high-dose interferon alpha-2b (HDI), an immunomodulatory cytokine. Evaluated in a randomized Phase 1 study (NCT01608594) led by Dr. Diwakar Davar at the University of Pittsburgh, the treatment is intended for patients with locally or regionally advanced or recurrent melanoma. Ipilimumab (Yervoy) enhances T-cell activation by blocking inhibitory signals from CTLA-4, while interferon alpha-2b (Intron A) exerts pleiotropic effects including enhancement of MHC expression and activation of natural killer cells. The regimen is administered prior to definitive surgery to achieve pathologic complete responses and improve long-term outcomes.
