Overview

A combination therapy consisting of **ipilimumab**, a monoclonal antibody checkpoint inhibitor targeting **cytotoxic T-lymphocyte antigen 4 (CTLA-4)**, and **rucaparib**, a small molecule **poly(ADP-ribose) polymerase (PARP) inhibitor**. Ipilimumab works by blocking CTLA-4 on the surface of T cells, thereby enhancing the immune response against cancer cells. Rucaparib inhibits PARP enzymes involved in DNA repair, leading to accumulation of DNA damage and cancer cell death, particularly in tumors with defective homologous recombination repair, such as those harboring BRCA1/2 mutations. The combination has been studied in clinical trials primarily for the treatment of relapsed ovarian, primary peritoneal, and fallopian tube cancer, as well as investigated conceptually for other malignancies. The combination is experimental and intended to leverage synergistic effects between DNA damage (via PARP inhibition) and immune checkpoint blockade (via CTLA-4 inhibition).
