A combination therapy regimen consisting of the immune checkpoint inhibitor ipilimumab and hepatic radioembolization using yttrium Y 90 glass microspheres. This sequential approach is specifically investigated for patients with uveal melanoma that has metastasized to the liver. Ipilimumab is a human monoclonal antibody that blocks cytotoxic T-lymphocyte-associated antigen 4 (CTLA-4), thereby enhancing T-cell activation and proliferation. Yttrium Y 90 glass microspheres are a form of selective internal radiation therapy (SIRT) delivered via the hepatic artery to provide localized high-dose radiation to liver tumors. The rationale is that radioembolization induces local tumor cell death and antigen release, which may synergize with the systemic immune-stimulating effects of ipilimumab to improve clinical outcomes in metastatic uveal melanoma, a disease typically resistant to standard immunotherapy.
